COPIES OF THESE ITEMS ARE REQUIRED FOR YOUR APPOINTMENT:

- Tax returns with W-2 or 1099 forms for the last two years
- Last 6 months of bank statements for checking, savings and/or 401K account
- Recent pay stubs (1 month), award letter from SSI, Child Support
- Social Security cards for all applicants
- Driver's license or state identification for all applicants
- Proof of citizenship or residency for all applicants

- Money order for $15 for each applicant payable to Centro Campesino
